Forever Life Group ("Forever Life Group," "we," "us," or "our") is a life insurance agency owned by Gerilyn Rodriguez (NPN 21265268), with an office at 5601 Powerline Rd, Fort Lauderdale, Florida 33309. Our work focuses primarily on Final Expense and Mortgage Protection life insurance for individuals, retirees, families, and homeowners.
Forever Life Group is an insurance agency. We are not an insurance company, and we are not a mortgage lender, bank, or loan servicer.
2. Scope of this policy
This Privacy Policy applies to personal information collected through our website, our online forms, our appointment booking pages, and our related communications with you by phone, text message, and email.
It does not apply to the privacy practices of insurance carriers, lenders, or other independent organizations. Insurance carriers that receive an application or quote request maintain their own privacy notices, which govern how they handle your information.

4. Health and sensitive information
Our public website and booking pages are not designed to collect detailed medical, health, or underwriting information, Social Security numbers, or financial account numbers. Please do not submit this type of information through website forms or message fields.
If you later decide to apply for insurance, the insurance carrier may require additional information to evaluate your application, which can include health history, medications, lifestyle information, date of birth, Social Security number, banking information for premium payments, and beneficiary details. That information is collected as part of the application and underwriting process, is used for that purpose, and is shared with the carrier and its service providers as needed. It is subject to applicable insurance privacy laws and the carrier's own privacy notice.

Mobile information, including text message opt-in data and consent, is not shared with third parties or affiliates for their own marketing or promotional purposes.
8. No sale or rental of information
Forever Life Group does not sell your personal information. Forever Life Group does not rent your personal information. We do not give your personal information to unrelated third parties so they can market their own products or services to you.
Sharing information with service providers that operate our systems on our behalf, or with insurance carriers when you request an insurance service, is different from selling data to marketers. Those disclosures exist only to run our business and to fulfill what you asked us to do.
9. Insurance-related disclosures
The insurance industry is subject to privacy laws and regulations, which may include federal law such as the Gramm-Leach-Bliley Act and state insurance privacy laws. When you apply for or obtain an insurance product, you may receive separate privacy notices from the insurance carrier describing its practices. Providing information to us, scheduling a conversation, or receiving a quote does not create insurance coverage. Coverage exists only when an application has been approved and a policy has been issued by an insurance carrier, as described in our Terms of Service.

We keep personal information for as long as reasonably necessary for the purposes described in this policy, including responding to your inquiry, maintaining our relationship with you, servicing policies, maintaining records of consent and opt-outs, and meeting legal, regulatory, tax, licensing, and insurance recordkeeping requirements.
